What changed on the PTCB exam in 2026

On January 6, 2026, the Pharmacy Technician Certification Board switched the PTCE to a new content outline. Three topics came off the exam, one new knowledge area went on, and every domain's share of the test moved. If you're studying from a book, app, or question bank written before then, part of what you're drilling won't be tested, and part of what will be, it doesn't cover.

- Removed: nonsterile compounding (two former knowledge areas), alligations, and narrow therapeutic index (NTI) medications as a standalone knowledge area.
- Added: a knowledge area on the Drug Supply Chain Security Act (DSCSA): product serialization, tracking and tracing, and quarantining suspect product.
- Rebalanced: Medications shrank; Federal Requirements grew the most; Patient Safety shrank slightly; Order Entry and Processing grew slightly.
Domain weights, old vs new
The PTCE still has four domains. What changed is how much of the exam each one is worth:

Federal Requirements is the biggest mover, up from one-eighth of the exam to nearly a fifth, and it's where the new DSCSA material lives. If your prep treated federal law as a small section to skim, that's the first thing to fix.
What was removed, and what it means for your studying
Nonsterile compounding used to be a tested topic. Compounding is still part of pharmacy practice, but it's no longer tested on the PTCE, so time spent memorizing compounding procedures now buys you nothing on exam day.
Alligations were a classic stumbling block in the math section, and a lot of older prep material spends pages on them. You can drop them. Calculations as a whole haven't gone anywhere, though: formulas, calculations, conversions, sig codes, and days supply are all still in the Order Entry and Processing domain.
Narrow therapeutic index (NTI) medications, the drugs where small dosing differences matter most, are no longer their own knowledge area. PTCB notes NTI information may still come up inside other areas where it's relevant, so don't drop it entirely.
What was added: DSCSA
The Drug Supply Chain Security Act sets out how prescription drugs are identified and traced as they move through the supply chain. On the new outline it has its own knowledge area inside Federal Requirements, covering:
- Serialization: the unique product identifiers on drug packages
- Tracking and tracing product as it changes hands
- Quarantining and handling suspect or illegitimate product

- 1.6 Indications of medications
- 1.7 Drug stability
- 1.8 Proper storage of medications
Federal Requirements (18.75%)
- 2.1 Storage, handling, disposal of hazardous and non-hazardous substances and wastes
- 2.2 Controlled substance prescriptions and DEA schedules
- 2.3 Receiving, storing, ordering, labeling, dispensing, returning, take-back, loss/theft, destruction of controlled substances
- 2.4 Restricted drug programs (pseudoephedrine, REMS)
- 2.5 FDA requirements for medication recalls
- 2.6 DSCSA serialization, tracking, tracing, quarantining
Patient Safety and Quality Assurance (23.75%)
- 3.1 High-alert/risk and LASA medications
- 3.2 Error prevention strategies
- 3.3 Issues requiring pharmacist intervention
- 3.4 Event reporting (MedWatch, VAERS, RCA, CQI)
- 3.5 Types of prescription errors
- 3.6 Infection prevention and cleaning standards
Order Entry and Processing (22.5%)
- 4.1 Formulas, calculations, conversions, sig codes, days supply
- 4.2 Equipment and supplies for drug administration
- 4.3 Lot numbers, expiration dates, NDC numbers
- 4.4 Returning dispensable, non-dispensable, expired medications
How to check whether your prep material is current
- Does it still teach alligations or nonsterile compounding as tested topics? It predates 2026.
- Does it cover DSCSA at all? If not, it's missing a whole knowledge area.
- Does it describe Medications as 40% of the exam? That's the old weight; it's 35% now.
A "2026" on the cover isn't enough on its own. Check the content against the outline above.
